Original video with showcase of Incident
Credits goes to Radiation Hazard

I think, this happend, because of HL2B save system. Game tried to load Autosave, but loaded latest save from that map, and started everything from the start.

Any other ideas?

I think that the NPCs might have attacked themselves just like the player, leading to them dying. that's my theory at least.  !dead

About the bullsquids, maybe it was the Headcrab that killed the bullsquid, although I have no idea, I never saw a Headcrab kill a bullsquid

About the blood on the walls, it may have been a Man-Hack who accidentally killed the metropolices

Most likely the whispers are most likely the Vortys, probably one of the nodes that he had to walk, blocked, and he was trapped in the building

Just watched the video and its pretty strange but as someone commented when he started shooting with the pistol near the end of the map and it hurt him it could've hurt the other npcs as well so possibly something wrong with the hitboxes.

I must agree to some of responders. Yes, because beta is laggy, it could be the reason. Hitboxes could move forward for a few inches, making NPC's hurt themself.

What is most likely happening is that due to a glitch in the guy's skill.cfg, all NPCs have 0 health, meaning that after performing a script they would usually die (remnant from HL1 scripts) you can also see in the video that everything he shoots dies in 1 hit. He also probably loaded an older save which is why some things are dead
