The DLCs that I have no idea about and have no documentation online are
650 - GameStop PS3 DLC - Seems to be a promotion for when you buy a ps3 or portal 2 for the ps3 at GameStop.
651-4 Bot X Y DLC - Just Bot Customization Options
654 - Summer 2011 DLC - I have no theories.
669 - Sixense Motion Preorder - Seems like Bonus Content for the now Offsale Sixense Motion Pack DLC.

I read up a bit on the Summer 2011 DLC. From what I gathered, the Summer 2011 DLC never released, but I think it just turned into the Peer Review DLC in October.
i read an article talking about the Peer Review DLC from 2011 (aptly titled: "Valve Didn't Forget About Portal 2's "Summer" DLC"). it doesn't mention the Summer update outside of the title, though. just the Peer Review DLC.
Now why it's separate from the Peer Review DLC, I have no clue. It might be that since the DLC was possibly delayed, they couldn't change it, so they left the ID up. Just a speculation.

Not necessarily. I read that there were pre-release bonuses for pre-ordering. I think that's what 651-4 were, since they were separate.
The gamestop Pre-Order bonus might be related to the pre-orders as well. Maybe there was a different bonus for Playstation (as to why SteamDB is documenting a Playstation edition, I don't really know).
The Sixense Motion Preorder doesn't seem to really have any information (might not've ever released). If it did release, my best guess was it either allowed players to play early, or maybe gave free Robot Enrichment cosmetics. Not quite sure on that one.
